Chapter Status

Rewards

You decide on the number of points required for mini rewards and major rewards. If you desire, you can even specify the number of points needed for each individual reward.

Mini Rewards

Possibilities include gift certificates to various stores, Healthier Days exercise kits, family safety kits, vacation time, cash, charitable donations, and anything else you want to offer.

Major Rewards

As with the mini rewards, the possibilities are endless — with the difference being that the dollar value of these "major" rewards would be higher than the mini rewards.

Financial Incentives and Support

  • Enrollment incentive (if program is voluntary)
  • Insurance rate adjustments based on participation levels
  • Contributions to HRA and/or other benefits
  • Payment for taking the Self-Check Health Check, an annual personal check-up that uses a self-report form to help evaluate the member's state of health. This SCHC is a nurse and physician developed HRA that we developed within HDC.
  • Financial support for participation in "Help Me" programs
  • Paybacks for co-pays and deductibles
