Implement any of these programs, and you will be helping your people experience Healthier Days:
Exercise
Our Exercise program is completely personalized and participant-driven. Participants create their own custom weekly program by selecting from a list of suggested forms of exercise and entering the frequency and duration of their planned exercise for each week. Participants can earn points based on how long they exercise each day (up to a predetermined maximum number of points). Healthier Days contains many articles and tip sheets to help participants get on the right track toward a healthy regular exercise routine.
Eat Healthier
Our Eat Healthier program is designed to guide participants through the process of living and eating healthy step-by-step.
The Eat Healthier program consists of 4 levels, each of which builds on the preceding level and includes slightly more in-depth diet and nutrition topics. Each level begins with a series of brief articles to help participants understand what eating healthy means, why it is important, and how to make healthy eating work for them. Each article is followed by a series of actions that are automatically placed on the participant's Action Record to help reinforce what they have learned. Participants may be asked to journal about their thoughts on the week’s article topic, plan their meals and snacks for the day, or cook their favorite healthy recipe.
The actions that are placed on participants' Action Records are personalized based on individual preferences. For example, participants who are allergic to nuts will not receive any actions that involve nuts and vegetarians/vegans will not receive any actions involving meat. Also, the Eat Healthier program will prompt participants to take action on specific areas of their diets which they can choose to focus on – such as eating more fruit, vegetables or whole grains or eating less fast food.
Help Me Manage My Weight Risk
This program is designed to help participants meet their weight loss goals through healthy lifestyle changes.
With the guidance of a health coach, participants in our weight loss program:
- Create and follow a personalized Action Record with specific daily Exercise and Eat Healthier tasks.
- Make regular entries in their online journal briefly describing their participation, the efforts they are making each day toward their goal, and any successes or struggles they might be having.
- Weigh-in weekly using our Weight Loss Tracker. Participants receive bonus points for each pound that they lose (above and beyond the points they earn on their Action Record).
A commercial or private weight loss program may also be utilized in conjunction with this program.
Help Me Quit Tobacco
As with other Healthier Days programs, the Help Me Quit Tobacco program is built on healthy lifestyle actions. Assisting those who would like to quit tobacco is a personal journey with moment-by-moment lifestyle choices. We provide the tools and assistance needed to help tobacco-users take the necessary steps to achieve a tobacco-free lifestyle.
Our methods are based on the latest scientific research and practical experiences of some of the nation's leading tobacco cessation researchers. Participants will learn how to properly treat their nicotine addiction as a disease.
Our Help Me Quit Tobacco Program has been customized into two options:
- "Talk, Read & Quit" participants receive telephone counseling, read selected articles, utilize medications as needed and track their progress. Separate program tracks are provided for smoking and for smokeless tobacco use.
- "Talk, Journal & Quit" participants do all of the above and also journal as another means to help achieve their goal of quitting. Here, too, separate program tracks are provided for both smoking and smokeless tobacco use.
Participants in both programs receive:
- Unlimited toll-free telephone access to their coach for one-on-one telephone treatment sessions. Participants will receive a series of phone calls, all scheduled at their convenience, that will assist them through planning and preparing to quit. Our coaches will guide participants all the way through the quit process.
- A personalized program geared to the issues they are dealing with and answers to their immediate questions. We will set up treatment sessions for participants and will provide individualized support and encouragement. We're here to help!
- Recommendations on type, dose, and duration of medications &/or Nicotine Replacement Therapy (NRT).
- Instruction on the different aspects of the addiction that will help participants to remain tobacco-free!
- Reimbursement for medications and/or NRT (if offered as part of the benefit).
Help Me Manage My Diabetes Risk
Help Me Manage My Diabetes Risk is both for those who have been diagnosed with diabetes as well as those who have been determined to be at a greater risk for developing diabetes.
Participants in our diabetes risk reduction program:
- Work one-on-one with their personal Health Coach to create a Diabetes Risk Reduction Plan of Action. This Plan of Action can be updated over time as needs and circumstances change.
- Decide what short- and long-term goals they would like to focus on and identify areas of their lifestyle that they need help with as well as those areas that they are already working to improve. Our Coaches will work with participants to determine the areas of their current diet that need to be focused on and guide them to make healthier choices in those areas. This involves a review of the appropriate number of servings of each food group that participants should strive for on a daily basis, as well as other healthy diet and exercise tips to help participants live an overall healthier lifestyle.
- Make regular entries in their online journal briefly describing their participation, the efforts they are making each day toward their goals, and any successes or struggles they might be having, etc.
- Check in with their Health Coach at least once a week to report on their progress and receive guidance and advice.
Help Me Manage My Heart Risk
Help Me Manage My Heart Risk is designed to educate participants about the heart disease process and help establish preventative techniques that will enable them to have a stronger and healthier heart.
Participants in our heart disease risk reduction program:
- Work one-on-one with their personal Health Coach to create a Heart Disease Risk Reduction Plan of Action. This Plan of Action can be updated over time as needs and circumstances change.
- Are guided through short informative articles that will help them to determine a course of action in their daily health habits. Together with their Coach, participants will focus on three areas of concentration: exercise, nutrition, and stress management. The goal is to show how these affect the cardiovascular system and what lifestyle changes participants can make to improve their cardiac status. As participants move from one area to the next, they will continue to build upon the previous topics.
- Make regular entries in their online journal briefly describing their participation, the efforts they are making each day toward their goals, and any successes or struggles they might be having.
- Check in with their Health Coach at least once a week to report on their progress and receive guidance and advice.
